BEFORE YOUR APPOINTMENT

Preparing for Your Consultation

Before we meet

A little preparation before your appointment allows us to make the best use of our time together.

Before your initial consultation, you will be asked to complete a detailed intake form covering your health history, current concerns and treatment goals.

I personally review your completed intake form and any relevant health records before we meet. This preparation forms part of your consultation service and allows us to spend more of your appointment clarifying important details, exploring relevant patterns and discussing appropriate next steps rather than starting from scratch.

Completing your intake form

You will receive an email containing a secure link to your intake form.

Please allow approximately 15–20 minutes to complete it and provide as much relevant detail as you can. You do not need to have everything figured out—the form simply helps me understand your health history and identify where we may need to explore further.

Your completed intake form must be submitted at least 48 hours before your appointment to allow adequate time for clinical review and preparation.

If your intake form is not received by this deadline, your appointment will need to be rescheduled.

Providing health records

If you have any relevant pathology results, blood tests, imaging reports, specialist letters or other medical records, please email them to the clinic before your appointment so they can be reviewed alongside your intake information.

YOUR CONSULTATION

What happens during your consultation

Your consultation is a dedicated, structured conversation focused on understanding your health history, current concerns and priorities.

Depending on your individual needs, we may:

  • Explore your symptoms and health history in greater detail

  • Identify factors that may be contributing to your current health concerns

  • Review relevant pathology, medications and supplements

  • Discuss whether further testing may be clinically appropriate

  • Begin developing a personalised and achievable treatment direction

You do not need to have everything figured out before your appointment. The consultation is a collaborative process, with time for thoughtful discussion, questions and clinical reasoning.

Online consultations

Online consultations are conducted through a secure video link. Your consultation link will be emailed to you before your scheduled appointment time.

Your treatment plan

Following your consultation, you will receive a written treatment plan and prescription where appropriate.

Your plan will bring together the priorities discussed during your appointment, your initial recommendations and the next steps in your care.
