Preparing for your Consultation

Sara Judd Naturopathy | Clinical Naturopathy

To ensure your consultation is clinically meaningful and focused, all new clients are required to complete an intake form prior to their appointment.

Please allow approximately 15–20 minutes to complete the intake form and ensure all questions are answered in as much detail as possible. The information you provide allows me to review your health history thoroughly and prepare appropriately for our consultation.

To allow adequate time for clinical review and preparation, intake forms must be completed at least 48 hours prior to your appointment.

If intake forms are not completed in advance, we will be unable to proceed with the consultation.

For security reasons, intake form links expire after 14 days, so I recommend completing your form as soon as possible once you receive it.

Halaxy & the patient portal

Halaxy is the secure clinical system used to manage your care.

Through your Halaxy patient portal, you can:

  • complete and save intake forms

  • upload documents securely

  • access your treatment plans and prescriptions

  • view invoices and receipts

To complete your intake forms and access your care, you will be prompted to create a secure Halaxy patient account. You may also receive a one-time authentication code by email to access the portal securely.

If you have any relevant pathology, blood tests, imaging, specialist letters, or medical reports, please upload them to your Halaxy account prior to your appointment and ensure they are shared with your practitioner.

To upload documents, go to My Health, select Notes, and choose Upload file. Once the file has uploaded, please click on the document and ensure it is shared so it can be viewed prior to your consultation.

What happens during your consultation

Your consultation is a dedicated, structured session focused on understanding your health history, current concerns, and goals.

During your appointment, we may:

  • explore your health history and presenting concerns in detail

  • identify key factors influencing your health

  • discuss appropriate testing where clinically indicate

  • begin developing a personalised plan aligned with your needs and priorities

Consultations are paced to allow for thoughtful discussion, clinical reasoning, and clarity. There is no expectation to have everything “figured out” before your appointment. The consultation is a collaborative process.

Telehealth consultations are conducted via a secure video link. Your consultation link will be sent to you via email prior to your scheduled appointment time.

After your consultation

After your appointment, you will receive a written treatment plan and prescription where appropriate.

Your treatment plan and any prescriptions will be uploaded to your Halaxy account, where you can access them at any time.

Please allow up to 48 hours following your consultation for your treatment plan to be finalised and uploaded.

Communication

All communication, document sharing, and follow-up occurs through the Halaxy patient portal. This ensures your information remains confidential and your care is managed in an organised and secure manner.

Please use the Halaxy portal for any questions related to your care rather than email or social media.
